I have seen this several times. Usually its on my Xen hypervisors running with PERC 5 while running with high I/O.
Things I have done to mitigate the issue include: - installing the latested megaraid_sas/mptlinux drivers - updating the PERC bios to the latest available 4/5 times this has solved my problems. I think most likely due to the fact that my Xen instances are running 2.6.18. 2.6.18 includes a fairly old megaraid_sas driver. One time, however, I did end up replacing the DIMM on the PERC card. In addition to seeing the "I/O rejected for offline device" messages, I got memory errors upon reboot. The hot swap drives weren't blinking, there doesn't appear to be anything wrong, I rebooted the server and I can now write to the filesystem again. Any ideas? thanks, -Drew
