Can't tell you *what* the problem is, but I can tell you *where* it is.
Fairly early in the log listings you sent, you see these lines:

>Jul  2 23:58:17 localhost pppd[1007]: Connect: ppp0 <--> /dev/cua3
>Jul  2 23:58:42 localhost PAM_pwdb[695]: (su) session closed for user root
>Jul  2 23:58:47 localhost pppd[1007]: LCP: timeout sending Config-Requests
>Jul  2 23:58:47 localhost pppd[1007]: Connection terminated.

This (the thrid line, specifically) means that you and the ISP failed to
negotiate the LCP protocols that ppp needs for a connection. I think this
means that the ISP isn't responding at all to the LCP requests, but I may be
misreading the message in that respect. 

In any case, to see what is actually happening during LCP negotiation, you
need to run pppd with the "debug" and "kdebug 0" options set, then check in
/var/log/debug (assuming you have a normal syslog setup) for the listing of
the LCP exchange between you and the ISP.
