This looks like the typical output for the "raid module not in
kernel" problem; I'd say raid5 is compiled as a module and not
included in an initrd.
Solution:
a) get a kernel including the raid code right in the kernel image or
b) make an initrd containing the raid module. both pretty hard if
this is your only machine.
