It could, especially if raidtab is set incorrectly.

Raidtab can get out of sync with reality if you're using
raidhot{remove,add}. You should check /proc/mdstat after the resync from
hotadd and ensure raidtab matches reality.

It should. When I've done raidhotadd the new device came up as device
/two/, but adter the resync, it was pushed back to 0 or 1, as appropriate.
Still... its worth a check.
