No mkraid output, no /proc/mdstat contents, no syslog entries...
Please send those.
Since it seems correct 80%+ of the time, I'll guess your /proc/mdstat
output includes md[0-3] and you're running an unpatched kernel
for 2.2.{11,12,13}:
