Requests are pending, see above output. My assumption/theory was that
we were racing on setting restart. Simplified version:

CPUX                                    CPUY
get_driver_tag, fails
                                        complete request, put tag
                                        check restart, not set
mark needing restart

and stall unless new IO is started on the device, since we now have two
requests waiting to be dispatched, and a driver tag available for one of
them, but nobody left to run the queue.

I wasted most of today bisecting this, only to discover it is present in
earlier kernels too. So I have nothing backing up the above, except the
fact that:

1) We have requests ready to issue in 'dispatch'
2) We have a driver tag available
3) hctx is marked restart, but looks like that happened later since no
   driver tags are pending

I'll try and debug this tomorrow.

As I said, this isn't necessarily the same problem that Bart is seeing,
since he doesn't trigger it on 4.14-rc or earlier, only in
for-4.15/block. Or it could be the same, just a wider window now.
Another data point is that I can't trigger it with scsi_debug, but it
triggers pretty easily with null_blk.
