where is your tape connected to? Your internal controller or
ditto dash? You did not configure the irq, so ftape will use
the default value corresponding to address 0x3f0 which is irq

BTW you have limited data rate to 500kbps. With this value
ditto floppy tapes will never work. Ditto 2GB needs at least
1000kbps, other 2000kbps (AFAIK). If your onboard fdc does
not support higher data rates you definitely need a ditto
dash.
I use a ditto 2GB connected to a ditto dash and it performs
quite well. On big backups I get a data rate of about
13MB/min as total average.
