I experienced a similar effect with my newly-repaired Ditto Max.

The log messages were identical albeit ftape finally gave up with an
error code which ultimately produced Input/Output error.

How did I fix it?

1) Put ft_tracings=0,0,0,0,0 into /etc/conf.modules
   It seems the act of reporting ftape operation can affect ftape
   on slow systems.

2) Execute swapout 16 before the first tape operation which causes
   the ftape modules to be auto-loaded.

3) Perform backups after re-cycling power on the PC
   Probably only applicable to Iomega controller cards (they do seem
   to get confused at times).

N.B. Don't forget, with ftape you should be using ftmt not mt.
