I got dualhead working by using the following xorg.conf entries:
Section "Device"
Identifier "gpu"
Driver "opentegra"
EndSection
Section "Monitor"
Identifier "LVDS-0"
EndSection
Section "Monitor"
Identifier "HDMI-0"
Option "LeftOf" "LVDS-0"
EndSection
Section "Screen"
Identifier "Screen0"
Device "gpu"
Monitor "HDMI-0"
SubSection "Display"
Modes "1920x1200"
EndSubSection
EndSection
HDMI is the primary screen in this case.
I hope this helps.
