I'd be surprised if usb_kill_urb() turns out to be the problem. (Of course, as it's author I would say that!) After all, the "control timeout on ep0out" message comes from a handler that gets invoked only for URBs submitted using usb_start_wait_urb(), which always uses asynchronous unlinking. And even if it didn't, the unlinking part of the pathway has hardly changed. A more likely possibility is that for some unknown reason the ohci_hcd driver isn't receiving the interrupts needed to retire the unlinked URB.
