I'm trying to use the usb on an Accelent system. Can you point me to where
I should look to check zone0/1 allocations to ensure they're not broken?
After pci-sa1111.c, soon after the host controller tries to set the address
of a new device (which times out), there is a kernel panic.

If you are getting these warnings then you have a serious problem and
the hardware will not work.
I am guessing, but if you are seeing these messages you must not be
using the pci_map/unmap_single routines in arch/arm/arch-sa1100/pci-sa1111.c
I'm not sure how you managed to do that, but you *must* use pci-sa1111.c
with the SA1111 usb host controller.
What platform? if you changed the zone0/zone1 allocations you might break
the GFP_DMA region...
