Seems like Cyrix and PCI have ongoing problems, in all incarnations, which
seem to show up a lot with OHCI addon cards. If you use an Intel 486,
does this same problem show up? Some Cyrix-related problems have been
been known to vanish with just a CPU swap. (I know that may not be an
answer you want, or can easily test.)

Well, if one of them works, the others should ... you only mentioned a single
failure mode, it's possible there's only one (hardware-related?) bug:

Which means that the OHCI controller is probably not getting interrupts
to pass on to the camera driver, or if it's getting interrupts then the Cyrix
PCI issues may be rearing their ugly heads.
