I'll have a look at it.  Two things struck me as a bit odd about your
dmesg output (edited here), neither of which is supposed to happen:

> usb.c: USB disconnect on device 2
> # Disconnect mouse
> hcd.c: (no bus?): hcd_unlink_urb fail -22

happened the first time, not the second time.

> ohci-q.c: urb c393acc0 TD 2 of 3, patch ED

Happened the second time, not the first time.

Could you forward me (not necessarily on-list) the dmesg
bits relating to the "ohci-hcd" driver starting up?
