There was such a driver stack back before the 2.2.8 kernel settled the question by merging "Linus' Alternative". It was more monolithic, as I recall. People were just really starting to learn about USB.

Things like hotplug certainly support USB, like the usbfs-based user
mode drivers do, and both are outside the kernel.
