Looks can be deceiving.  From your log, either you didn't have usb-storage 
debugging configured on or else your syslogd configuration doesn't capture 
debug-level messages.  So you might be missing some important information.

You could be seeing the unhappy result of usb-storage's attempts at error 
recovery.  In particular, there might have been a usb_reset_device() call.  
That could account for a lot of peculiar behavior.
