When I now plug in a USB device to the USB2.0 card nothing happens. No device detect, not hotplug event, not even a interrupt on IRQ9 which I would have expected.
I see two wierdnesses. First, that "lspci" showed your card twice; which means that its PCI setup is broken. Second, that you didn't have "usb-ohci" loaded.
Root hub operations don't generate IRQs on Linux.
