the mouse will not disappear from /proc/bus/usb/devices, even if I unplug the mouse. Same for the keyspan device, it seems the ports are dead for some reason.
Sounds more like "khubd" is wedged. It's annoying, but that's a single-point-of-failure for the entire USB stack. Alt-SysRq-t is often informative at such times.
