These sorts of errors are usually caused by bad hardware, such as 
low-quality USB cables.  Often the culprit turns out to be a cable inside 
the computer case connecting the USB port to the motherboard.  But 
sometimes the device itself is at fault.

To get more information, turn on CONFIG_USB_DEBUG and see what appears in 
the dmesg log.
