What happens if you build usb-ohci.o with DEBUG enabled ... are you seeing that the interrupts get to that controller? Or maybe simpler, can you move the Ethernet and USB controller so they don't share an interrupt? Some folk have reported problems with usb-ohci and Cyrix setups.
