My fix for this error was not software.  I am embarrased to admitt it.
When I hooked up the external USB connectors, I accidentally inverted the
+ and - leads.  This allowed the USB drivers to initialize, but, when a
USB device was plugged in, it could not accepts a new address.  When I
connected my external leads correctly my USB issues went away.  Perhaps
sharing my silly error will help someone else get it fixed faster.

I sent this in as a suggested addition to the faq.
