Sounds like a power problem, but I don't know why Win2000 wouldn't trigger it.
The reason why I think a power problem is that it is connecting, drawing too 
much current, dropping the power supply rails and being disconnected. That 
would normally be shown in the logs.

Do you have a powered hub that you can try?
