There are a whole set of freely available specifications for devices (eg USB printers, USB input devices, USB mass storage devices - see http://www.usb.org) However some devices have ugly specs (eg Communication Device Class) that manufacturers didn't always like. So manufacturers implemented USB modems without controllers (because it was cheaper, and it worked with windows). Also, some manufacturers didn't want to be in the commodity market, and went for product differentiation.
