i've got quite the same problem on different hardware and kernel. i've got a debian system, "testing" branch, with an asus A7V8X motherboard (usb-uhci), a 2.4.20 kernel and a 5 in 1 card reader. my 128 MB sandisk SD card worked before, i could mount, read and write it. but one day, when i was copying a file to it, the whole machine froze. since then using the SD card, e.g. fsck'ing, blocks, the process goes into D state and cannot be killed. fortunately, the machine doesn't freeze when this happens.
