Those appear during normal operation, together with a lot of other stuff,
each time a command is sent to the device.  If you turn usb-storage 
debugging back off again they won't be generated.

Both the log excerpts you included look perfectly normal, except that the 
second (coldplug) one was apparently cut short.  Apart from that I can't 
really say anything, since it all looks correct as far as it goes.  What 
happened after that last "Attempting to get CSW..." message?  Even if 
there was no response from your device the command should have been 
aborted after a few seconds.
