There have been problems like yours with the EHCI driver reported recently, involving BIOS handoff of the controller to the operating system. A few patches have been posted on linux-usb-devel and LKML; I don't know what the final story is. If you haven't done so already, you should make sure that BIOS legacy USB support is turned off since it's a potential source of interference. Another thing you could try doing is comparing the behavior of different kernel releases. If you can find the last version which worked okay and the first version which failed, that would give the developers a good place to look for incriminating changes.
