Having eliminated some possible sources of errors, you've made a good start. The next step is to turn on the usb-storage verbose debugging option in the kernel configuration. You'll also have to edit /etc/syslog.conf to make sure that kernel debug-level messages are captured to a log file. These messages will provide more information about what's going wrong.
