According to the logs you sent, the ehci-hcd driver was never loaded.  The 
computer had a PIIX4 chipset, which does not include an EHCI USB 
controller.  Obviously the system won't use the ehci-hcd driver unless it 
is loaded and the hardware includes an EHCI controller.
