I can't explain all of the problems you see, but it's clear that the USB 
interface chip in your adapter case doesn't work very well.

Part of the problem is that it reports that it supports SCSI rev 04, but 
then it crashes when it gets a REPORT LUNS command -- for which support is 
mandatory in SCSI 04.  A patch to work around this was posted at

This doesn't explain the difficulty you have with mke2fs.  At this point 
the only thing I can suggest is to get a better enclosure.
