Alan, I believe, I never showed dmesg output with CONFIG_USB_DEBUG flag enabled when USB is inserted and cannot get mounted. The file is attached. Once USB device failed to mount, it is always hopeless to try to insert it again in this or another port (I have 4) until the computer is rebooted. After that it might or might not work again for a while.
