Thank you so much for the responses. I checked out the FAQ at http://www.linux-usb.org as suggested by Stephen Gowdy. It seems my problem had to do with a defective USB2.0 cable. The checks outlined in the FAQ in the 'device not accepting address' section were very helpful.
