Often these errors are caused by problems in communication, i.e., bad USB 
cables or connections.  (Sometimes the bad cables are the ones inside the 
computer case, going from the USB port to the motherboard.)

Sometimes the errors are caused by problems in the device itself, but that
tends to be relatively rare.  To get more information you can try using
the usbmon facility (see the instructions in the kernel source file
Documentation/usb/usbmon.txt).

Occasionally the problems are caused by the USB controller on the 
computer.  Not much you can do about that except to replace the hardware.
