If you have a CBI mass storage device attached to a UHCI controller with
uhci.o and usb-storage.o loaded, and then do the following:
(1) rmmod usb-storage
(2) insmod usb-storage
The message "uhci: host controller halted. very bad" will appear in the
logs. The CBI device will not function, nor will any other mass storage
devices attached to that controller. This is with 2.3.99-pre3-1.
Note that this procedure works just fine with usb-uhci.o on the exact same
machine. It also works with usb-ohci.o on (as you guessed) an OHCI
controller).
I can perform more post-failure tests if desired, but I generally reboot
at this point to recover the system.
