Many thanks to David Brownell who suggested I check /proc/interrupts
when using USB devices on my Supermicro motherboard.  Unsurprisingly,
it appears that the usb hardware isn't generating interrupts when
plugging/unplugging usb devices.

As I was getting a bit desparate, I decided to install Win98 and see if
it could use the usb devices.  Again, no usb devices worked.

So it looks like a hardware fault.  I don't know whether this is specific
to the Supermicro 370SD motherboard or to all i810 chipsets (the latter
is probably unlikely as other people probably would have come across this
problem by now).

Anyway, hope this info is useful for others.
