Various minor cleanups to resource init and free code including two
actual memory leaks. These should be applied after the WUSB STALL patch
I submitted earlier.

fix leak of urb in wa_xfer_destroy.
Use usb_init_urb to init the transfer segment URB in wa_seg_init
instead of initializing it manually.
