I'm also getting this with 3.19.5:

This could be a regression in 3.19 as I've never come across this
before. It happened on the first boot with 3.19 and I immediately
reverted to 3.18 (this is my main machine) so not sure if it happens on
every boot.
This is with an "Intel Corporation 8 Series/C220 Series Chipset Family
USB xHCI (rev 04)" controller and the external USB 3.0 hub in my monitor
(0451:8042 Texas Instruments, Inc. Hub):

I don't seem to be able to reproduce it on a different machine (but
perhaps I didn't try hard enough if it is indeed intermittent).
