That's if you want to add those patterns sytem-wide. For a per-user basis, use ~/.spamassassin/user_prefs as previously mentioned. Any perl-capable person for extending these patterns to body checks, a bit like scoring in procmail? Like for each match, add 0.1 (or so). This *must* be scope-limited, or you're putting a potential DoS on. Versions around 2.11 of spamassassin suffered from this - a 5MB email would take the system out.
