Thanks Ross for trying, but you're in the wrong forest. The problem has
nothing to do with headers, I mentioned to use their baseline as a
convenient visual reference point. The problem has to do with TeX's way
of assembling vertical material and the addition of weight-space above
or with the top vertical element, I suspect by means of giving a height
far in excess of the thickness of the \hrule, resulting in the black
line being lower than the top edge of the page body. The header is
outside the page body and therefore unrelated to the problem.
