Andrew has put me on the right track - using kde autostart does the job. Icewm does not appear to have the ability to run an app automatic
Create a script in ~/.icewm called startup (and make it executable). It will run when the user is logged in. Check out the other files in ~/.icewm. You can lock IceWM down so it doesn't do anything but run your startup script. I have 4 LTSP terminals running IceWM with a startup script that runs Firefox in kiosk mode and connects to our Intranet. If Firefox is closed it will automatically restart.
