Last time I did this, I set up a dedicated ssh daemon, and used the 
ForceCommand directive to point to a shellscript that basically just
did...

while true
do
        $Echo -n "."
        sleep 10
done

My view was that this was the simplest and safest option. These days,
I'd probably use openvpn instead (:
