Well, I can't help much.  But it looks to me like this is complaining about
the device with major 16, minor 40.  This site:

http://www.lanana.org/docs/device-list/devices.txt

says that's a Goldstar CD-ROM.  You might check a more recent devices.txt if
you have the kernel source handy (in linux/Documentation/devices.txt).  Or
you might check your /dev directory to see what has those numbers.

Seems to be a problem reading the install CD.  Do you have an unusual
CD-ROM?  Perhaps there's a way to tell SuSE what driver it should be using?

I hope I'm close enough not to be misleading...
