glibc's mnemonic.ds

[Marcin 'Qrczak' Kowalczyk]

It seems based on RFC1345 but has more mappings. Is that some another
standard, or private glibc's inventions?

PS. <"`>                   <U0300> COMBINING GRAVE ACCENT
should be probably <"!>, as ` is not used otherwise.
