Kindly check the auth_param lines of your proxy server.

auth_param ntlm program /usr/bin/ntlm_auth --helper-protocol=squid-2.5-ntlmssp
auth_param ntlm children 100
auth_param ntlm max_challenge_reuses 5
auth_param ntlm max_challenge_lifetime 10 minutes
auth_param ntlm use_ntlm_negotiate on
auth_param basic program /usr/bin/ntlm_auth --helper-protocol=squid-2.5-basic
auth_param basic children 50
auth_param basic realm XYZ Industries Web Access
auth_param basic credentialsttl 2 hours

correct it if there's any mistake in your conf file.
I have done integration of squid and samba with NTLM authentication.i'll send u 
the documentation.
Are the clients in a domain? and are u using ADS for authentication.?

            Is there any way to authenticate client users with proxy

server(ncsa, ntlm,pam, ldap etc) and the proxy been in the transparent

mode. I created a transparent proxy but then the auth_param function

doesnt works and the clients machine gets connected without

authentication.
