There is no problem with u r qmail setup, the remote server didn't respond with an SMTP error after sending message from your qmail server. If it's the problem with your qmail server than remote SMTP server should reply with an error code of SMTP standard i.e 440, 441,510 .... It might be a network problem. A common one is mismatched MTU Please use tcpdump or ngrep command to check the entire conversation between your qmail server and remote SMTP server with which u r facing the problem. Do you have any kind of attachment blocking service like simscan or qmail-scanner. Additionally also take a look at the /var/qmail/control/ databytes file. This is used to control attachment size limit on qmail.
