Remove NEW_DOC_CODE

Ron,
        I am trying to make doc_mil_fill_inbuf.c to use Eric's new
stream interface. Do you mind me removing the NEW_DOC_CODE part ??
AFIAK, the OLD_DOC_CODE does not have any problem.
