The custom iso is very easy to make. How much time it takes depends on the kind 
of customization your looking for. On a Ubuntu / Kubuntu  based system , you 
can use remastersys or reconstructor.

Thats
a how-to on remastersys to get you started. The only thing to remember
is that remastersys will automatically include all packages installed
on your system into the custom distribution. So its best to use it on a
new install. That will prevent installation of unrequired software.

This is a good tool as well. Has some more customizations.

The best way is to make a Live Distro from scratch. That way you can keep it 
quite trim by removing all
unnecessary packages. You can also have a different window manager if
you want to keep it really small.

There are also tools to customize fedora , knoppix, slax , DSL etc. 

Another tool worth mentioning is Insta Linux

Its
a online app where u can choose what apps you want. It generates a
custom cd for you on the fly which you have 24 hours to download /
distribute. The avaiable applications are quite limited though.
