IIRC, somewhere in the qmail docs it is mentioned 
that inetd sucks under high loads and hence tcpserver
is recommeded. Though I could not make much sense
out of it neither did I have the load to actually
check
it. Some performance metrics were also given. 

qmail however works beautifully under tcpserver.
