We take cards over the 'net amd we've never had a chargeback (knock on wood). We require matching address and zipcode for users in the US (unfortunately we can't turn it on for foreign card-users because many cc vendors in those countries don[t properly match the information), and require the CVV/CVS info (those three digits on the back). No match, the info comes to us for manual vetting. We've never bothered to match thelocation to the IP# because it's sometimes incorrect, and makes it hard for people not near home to use their card. And we also honor all comoplaints with complete refunds. Easy for us to do because we're not delivering tangibles, but it might be prudent for everyone, since if you contest it'll go to chargeback, and if it goes to chargeback it's going to cost you circa $20. And more than a few chargebacks you'll find it hard to find a merchant bank who'll let you keep accepting cards.
