I have so far done only up to clamav (of course modified a few steps as per new version and my requirement) I am still checking on Spam control options. I see that you have used qmailscanner in later stage. I was trying to avoid it and directly make qmail call spamassassin. Something similar to use of spamassassin in sendmail. We want to keep qmail untouched as far as possible.. since our developers have modified delivery part to suit our filesystem method. Also... the most important...
