Let's figure out what's wrong on your system.  First of all, don't
play with "unplug and replug" tricks.  Linux, or more accurately,
Xorg, doesn't support plug-n-play yet.  Graphire4 was released way
after Kernel 2.6.9 was out. Since you are running kernel 2.6.9, you
need to build hid-core.c on your system so USB HID driver will ignore
Wacom devices.  Looks like you might have built and installed it (your
output from " cat /proc/bus/usb/devices " is right).  If so, the
problem could be in Xorg.

Please issue "xsetwacom set stylus debuglevel 10" and   "xsetwacom set
stylus commondbg 10" then briefly move your stylus on the tablet and
email the part related to Wacom in /var/Xorg.0.log to me directly, not
to the list.  We'll go from there.
