I think there was a tag in the Allaire gallery to do something like this! I
can not get to the site to check it right now but I am pretty sure I saw
something like that.

Subject: Generic form tool?
Does anyone have or know of a tool that will let you submit forms w/a
varied number of form fields, and types to a DB? The idea is for someone
w/no programming experience to be able to create a form in a WYSIWYG and
point it to an action page to insert the info into a DB. I have a number of
ideas on how to do this, but I'm really interested in knowing if a tool
like this exists b/f I go down that road.
